AFFA Diagnostic & Treatment Medical Center (“AFFA,” “we,” “us”) operates affaclinic.com. This policy explains what information this website collects, how we use it, who we share it with, and the choices you have.

This policy covers our website, not your medical records

This policy applies only to information collected through affaclinic.com. Health information you give us as a patient, including at appointments, by phone or through our patient systems, is protected under federal law (HIPAA) and is covered by our Notice of Privacy Practices.

Please do not send medical details through this website. Our contact form and email are for scheduling and general questions. To discuss symptoms, test results or treatment, call us at (718) 880-7000.

Information we collect

Information you give us

  • Contact and appointment request form: your name, email address, phone number, insurance plan, the service you are interested in, your preferred day and time, and your message.
  • Newsletter sign-up: your email address and any other details the sign-up form asks for.
  • Calls and emails: anything you choose to tell us when you contact us directly.

Information collected automatically

  • Usage and device information: pages you visit, how you arrived at the site, approximate location based on IP address, browser and device type, collected through Google Analytics.
  • Spam protection: our forms use Google reCAPTCHA, which collects information about your device and how you interact with the page to tell people from automated programs.
  • Server logs: our web host records IP addresses, dates and times, and pages requested, to keep the site secure and working.

How we use information

  • To respond to your questions and schedule appointments.
  • To send an automatic email confirming we received your request.
  • To send our newsletter, if you signed up. Every newsletter includes an unsubscribe link.
  • To understand how visitors use the site and improve it.
  • To protect the site and our forms from spam, fraud and misuse.
  • To meet legal obligations.

We do not sell your personal information, and we do not use this website for targeted advertising.

Who we share information with

We share information only with service providers that help us run this website, and only as needed for the purposes above:

  • Google (Analytics, Tag Manager, reCAPTCHA, Maps and Fonts). Google Privacy Policy
  • MailerLite, which manages our newsletter list. MailerLite Privacy Policy
  • Font Awesome, which provides icons used on the site.
  • Automattic (Akismet), which may screen form submissions for spam. Automattic Privacy Policy
  • Our web hosting and email delivery providers, which store the site and deliver its emails.

We may also disclose information when required by law, to protect the safety of our patients, staff or the public, or as part of a sale or transfer of our practice.

Cookies and similar technologies

Cookies are small files a website stores in your browser. This site uses:

  • Google Analytics cookies (named _ga and _ga_ followed by an ID), which tell repeat visits apart. They last up to two years.
  • Google reCAPTCHA cookies, used for spam protection.
  • Login cookies, set only for our staff when they sign in to manage the site.

You can block or delete cookies in your browser settings; the site will still work. You can also opt out of Google Analytics with the Google Analytics opt-out browser add-on. This site does not currently respond to Do Not Track signals.

How long we keep information

  • Contact form submissions: kept in our website system and our email for one year, then deleted. If you become a patient, information needed for your care becomes part of your medical record and follows our record-keeping obligations.
  • Newsletter sign-ups: kept until you unsubscribe.
  • Analytics data: kept by Google for up to 14 months.

How we protect information

The site uses an encrypted (HTTPS) connection, and access to submitted information is limited to staff who need it. No website or email system is completely secure, which is one more reason not to send medical details through the form or email.

Your choices

  • Ask what information we hold about you from this website, or ask us to correct or delete it.
  • Unsubscribe from our newsletter at any time using the link in any email.
  • Limit cookies through your browser settings or the Google Analytics opt-out add-on.

To make a request, contact us using the details below. We may need to verify your identity before acting on it. Depending on where you live, you may have additional rights under state law.

Children

Many of our patients are children, but this website is meant for parents, guardians and adults. We do not knowingly collect information online directly from children under 13. If you are contacting us about your child, please fill out the form yourself. If you believe a child has sent us information, contact us and we will delete it.

Links and embedded content

Our site includes a Google Maps embed and links to other websites. Those services have their own privacy policies, and we are not responsible for how they handle information.

Changes to this policy

We may update this policy as our website or practices change. The effective date at the top shows when it was last revised.
